All nominations must include: nomination form, narrative summary, narrative and program samples.
1. Nomination Form
Select the appropriate award category - only one category can be selected for each entry. Duplicate entries will not be considered. Include the name of the person, group or program nominated. List the company or organization name and a contact person, along with the mailing address, email address and phone number. Printed nominations must include a printed nomination form 
.
2. Narrative Summary
Write a 150-word summary that covers the key points of the 1,000-word narrative. Should your entry be judged a winner, the narrative summary will be used for promotional materials and the awards gala.
3. Narrative
Write a long narrative about how the nominee has made a difference in the health of Oklahomans. Your narrative should be written using the outline below. (Limit: 1,000 words.)
A. Description of person or program
B. Goals
C. Target group of people
D. Results / outcomes / achievements
4. Program Samples
Include samples to show the scope and quality of the program. (Limit: Four samples.)
