It's easier than ever to nominate a Champion. Champions of Health nominations may be submitted online, along with supporting electronic documents in Adobe Acrobat, Microsoft Word, HTML or text format. Reminder: Online nominations have the same requirements as printed nominations.

Please carefully review the nomination requirements before submitting your online nomination.
Both online nominations and printed nominations are judged using the same criteria. Clear communication and professional presentation are encouraged. It is recommended that the 150-word narrative summary and 1,000-word narrative be prepared, edited and spell-checked in a text document, and then copied and pasted into the appropriate fields on the online nomination form.
